Lead capture forms and surveys that feed your CRM

Ask the right questions once. Every answer is saved to the contact's record in EDCRM.AI and can start the follow-up without anyone retyping it.

Forms that create contacts

A form is the simplest way to turn a visitor into a lead. In EDCRM.AI you build it by placing the fields you need: name, phone, email, drop-downs, tick boxes and dates. You can put it on a page built in the platform, embed it on your existing website or share it as a link or QR code.

When someone submits, their answers are written straight into the CRM as a contact record. There is no inbox of form emails to work through and nothing to copy into a spreadsheet. Your team sees the new lead in the same place they manage every other customer.

Surveys and quizzes for longer conversations

A survey spreads questions over several steps, which feels lighter than one long form. It suits lead qualification, needs assessments and feedback after a service. A quiz adds scoring, so the person gets a result at the end and you learn how to group them. Both are useful when you need to know more about a lead before a salesperson calls.

How answers land in the CRM

Each question is linked to a field on the contact record. Standard fields cover name, phone and email. Custom fields hold whatever is specific to your business, such as budget, preferred branch, course of interest or vehicle model. Once saved, the answers can be used anywhere in the platform.

  • Answers appear on the contact record for anyone in your team to read.
  • Smart lists can filter contacts by any answer, such as budget range or location.
  • Messages can include answers, so a reply mentions the service the person asked about.
  • Source reports show which forms and campaigns are producing your leads.

Starting automations from a submission

A submitted form is one of the most useful triggers in the workflow builder. The moment the answers are saved, a workflow can acknowledge the enquiry, assign it to the right person and create a task. If/else steps read the answers, so a lead interested in one service follows a different path from a lead interested in another.

What to prepare before you start

Short forms are easier to complete than long ones, so the main preparation is deciding what you truly need to know at the first contact. Everything else can be asked later by your team or in a follow-up survey once the conversation has started.

  • List the questions your sales team asks on every first call.
  • Mark which of those are essential before the call and which can wait.
  • Agree the options for each drop-down, such as services, branches or budget bands.
  • Write the consent wording for receiving WhatsApp, SMS and email messages from you.
  • Decide who should be notified and assigned for each type of enquiry.

How Euro Digital sets it up

Our team builds your forms and surveys, creates the custom fields behind them and styles them to match your brand. We add opt-in wording for WhatsApp and other channels, which you should check against your own legal obligations. Each form is connected to a workflow and tested end to end, from submission to the message your customer receives, before it goes live. Your team is trained to edit questions themselves.

Questions

Forms and surveys: common questions

Can we put the forms on our existing website?

Yes. Forms and surveys can be embedded on pages of your current website or shared as a direct link. Your web developer, or our team, places the form on the pages you choose. Submissions go to the CRM in exactly the same way as from a page built inside EDCRM.AI.

How is a quiz different from a survey?

A survey collects answers. A quiz also scores them and shows the person a result at the end. Use a survey when you simply need information, such as requirements or feedback. Use a quiz when a result is useful to the customer and helps you sort leads into groups.

How do we handle consent for WhatsApp messages?

Add a clear opt-in tick box to the form stating that the person agrees to receive WhatsApp messages from your business. The answer is stored on the contact, and workflows can check it before sending an approved template. You should confirm the wording meets your own legal obligations.

Can leads from Facebook and Google adverts come in too?

Yes. The ads manager syncs leads from Google and Meta adverts into the CRM, so they arrive as contacts just like form submissions and can start the same workflows. For adverts that send people to a landing page, the form on that page captures the lead.
